Requirements

   Duly Accomplished and Notarized Locational Clearance Application Form
   Proof of Right-Over-Land

Note: In case the Property is not registered in the name of applicant, submit photocopy of the following:

   Duly notarized deed of sale/ deed of donation
   Contract of Lease; MOA, Usufruct of Agreement
   Affidavit of Consent / Authorization to use land, whichever is applicable
   Certified True Copy of Transfer Certificate of Title
   Certified True Copy of Tax Declaration
   Current Tax Receipt of Non-Tax Delinquency Certificate
   Barangay/Assessor's Certification if Lot is Not Titled
   CENRO Certification if Lot is not Titled

Note: In case the Property is registered in the name of applicant, submit photocopy of the following:

   Certified True Copy of OCT / TCT
   Tax Declaration
   Current Tax Receipt of Non-Tax Delinquency Certificate
   Bill of Materials

Note: The documents should be:

   Reflective of the total amount of construction cost of the project;
   Signed & Sealed by a duly authorized professional;
   Conformed by the applicant.
   Five (5) Sets of Building Plans
   Signed & sealed by Architect / Civil Engineer
   Vicinity Map with technical description & lot coordinates
   Site Development Plan

Prior CONSTRUCTION/EXPANSION/RENOVATION

   Distance/Clearance Requirements if applicable
   Road Right-of-Way Clearance from DPWH or PEO
   DENR Watershed Clearance (Protected / Non-Protected Area)
   DENR Waterway Clearance
   DAR Clearance/ Conversion Order/Exemptions (if the proposed site is Agricultural area)
   Environmental Compliance if applicable
   Environmental Compliance Certificate
   Certificate of Non-Coverage

Prior CELL SITE/TOWER

   Environmental Compliance Certificate (ECC) from DENR (if the proposed site is within an environmentally critical area)
   Land Use Conversion from DAR (if the proposed site requires conversion of an agricultural land area)
   Special Use Agreement (if the proposed site is a Protected area)
   Certification (from DPWH/NORSAMELCO), if applicable
   Height Clearance Permit from CAAP (except if the proposed site is outside the CAAP Critical area and below (50)meters in height above the elevation of the ground)
   Written Consent
If the proposed project is within subdivision
   Approval of the HOA and all members whose properties abut the proposed project site.
   In the absence of a HOA, consent/affidavit of non-objection from majority of actual occupants and owners of properties within a radial distance. Equivalent to the height of the proposed project including those abutting the proposed site.
On Buildings – occupied or unoccupied:
   Consent of the owner/developer
   Approval of the governing board/body including all tenants occupying the topmost floor directly below the proposed project and the consent of the owner and majority of tenants of adjoining buildings.

Prior POULTRY/PIGGERY/RICEMILL/FUNERAL PARLOR/SLAUGHTERHOUSE or ABATTOIR/COCKPIT

   Environmental Compliance Certificate (ECC) or Certificate of Non-Coverage (CNC) from DENR – EMB
   Conversion Order (CO) or Exemption Clearance (EC) from DAR
   Site Clearance from the Local Health Officer or Municipal/City Veterenarian concerned on the suitability of the location pursuant to Chapter IX of PD 856’s IRR on Nuisance and Offensive Trade and Occupation
   Barangay Clearance
   Barangay Resolution/Affidavit of No Objection from Adjacent Occupants

NOTE: Additional requirements may be required after inspection and evaluation of documents. All documents should not be more than 3 months old upon filing.
